Submit your Mandatory Immunization Health History Form and supporting documentation through the Immunization Status App in MyUWF. Late, incomplete, or inaccurate information may delay registration.

All AFJROTC students are required to submit a FERPA Release form allowing UWF to share your educational records with the Air Force Junior Reserve Office Training Corps (AFJROPTC), a Government sponsored program.

Financial Responsibility Statement

Complete the Financial Responsibility Statement, which is an agreement the University requires every UWF student to acknowledge before registering for classes. This can cause confusion for students whose tuition is paid by a third party. Unfortunately, there's no way to avoid this requirement, regardless of a student's method of payment.  Thankfully, it's an easy process!
