Once accommodations and services have been approved through the Student Accessibility Resources (SAR) office, students must submit a Semester Request Form each semester to access their accommodations. Accommodations are granted on a semester-by-semester basis to align with current course enrollment and to ensure that instructors are properly notified. Before submitting the Semester Request Form, students are encouraged to attend the initial class sessions and review the syllabus to determine the specific accommodations needed for each course.
Submitting a Semester Request Form (Student)
- Log into MyUWF
- In the search bar type “SAR”
- Choose the option that says “SAR Accommodate – Student Portal”
- The Semester Request form can be found under the “Accommodation” tab. (This tab is the second tab from the top of the screen on the left side, after “home” tab)
- Select the button labeled “Add New” (This button is located toward the left of the screen after the “Apply Search” button)
- Select the semester you are requesting to receive the accommodation from the drop down menu. (This menu is located to the left side of the screen)
- After you choose a semester, the screen will refresh and the courses you are enrolled in will be listed on the right hand side of the screen
- Select the button labeled “Review the Renewal” located below your course listing.
- The next screen will list each of your approved accommodations and each course in which you are enrolled. Review each accommodation to ensure the box is selected next to the course you are requesting to utilize the accommodation. If you do not want to utilize the accommodation for a specific course, ensure the box is unmarked.
- Once you have completed your review, select the “Submit” button located in the bottom left hand corner.
