Submit your UWF Immunization DocumentsAfter activating your MyUWF Account, submit your Mandatory Immunization Health History Form and supporting documentation through the Immunization Status App in MyUWF. Late, incomplete, or inaccurate information may delay registration. Please note: if you are under 18 years old, in addition to your signature, your parent or legal guardian needs to sign the Mandatory Immunization Health History Form as well. If you have not had the Meningitis or Hepatitis B immunization, you can waive both of those requirements in Section B of the form. If you choose to waive those requirements, and are under 18 years old, your parent or legal guardian will need to sign Section B as well. Immunization Questions?Student Health Services |
Complete the AFJROTC Student Information FormThe AFJROTC Student Information Form is a secure, electronic form which will be sent to your personal email address. This form is where you will share required information and upload required documents. NOTE: Expired documents, temporary documents, or documents that will expire during your enrollment period will not be accepted. Documents to be UploadedThe following documents are needed to successfully complete the form. You may need access to a scanner or scanner app (for mobile devices). PDF files are preferred.

Complete the Financial Responsibility Statement AgreementComplete the Financial Responsibility Statement, which is an agreement the University requires every UWF student to acknowledge before registering for classes. This can cause confusion for students whose tuition is paid by a third party. Unfortunately, there's no way to avoid this requirement, regardless of a student's method of payment.
