Late, incomplete, or inaccurate information may delay registration. Please note: if you are under 18 years old, in addition to your signature, your parent or legal guardian needs to sign the Mandatory Immunization Health History Form as well. If you have not had the Meningitis or Hepatitis B immunization, you can waive both of those requirements in Section B of the form. If you choose to waive those requirements, and are under 18 years old, your parent or legal guardian will need to sign Section B as well. This can cause confusion for students whose tuition is paid by a third party. Unfortunately, there's no way to avoid this requirement, regardless of a student's method of payment.
