1. Follow the link for the form, it will be sent in an email from your advisor. 
2.  Your name and student ID should be automatically filled in. 
3. Read the Admissions Requirements, check the box indicating your understanding.

4. Read Important Teacher Education Information and Deadlines, then check the box indicating your understanding. 

5. Read Other Acknowledgements, and check each box indicating your understanding.  Once you have checked all of the boxes, please click Next. 

6. Type your name to electronically sign. Once complete, click Sign Electronically.
