Enrollment Certification for Exchange and Transfer Credit Programs
The Enrollment Certification form is used by UWF to confirm a student has arrived abroad and has begun classes. UWF requests each student complete their personal information at the top of the form and submit to the host school or program international office after arrival. The host school or program office abroad may request the student to list their courses or modules on the form. UWF asks that each host institution or program's International Office verify the course dates for each class and complete the signature portion of the form before sending the form to UWF. The form should be submitted by email or fax to the UWF Registrar, preferably by e-mail attachment (pdf) to RegistrarExchange@uwf.edu or by fax, Attn: Registrar Exchange to 850.473.7345.
The Enrollment Certification serves as attendance verification for students abroad. Financial Aid cannot be disbursed until after the Signed Enrollment Certification is received by UWF from the host school or program abroad.
