Instructions
Step 1
You must first log into MyUWF. The form link in step 2 below will not work unless you are first signed into MyUWF.
Step 2 - Click HERE to access the EdD in C&I - Degree Audit Checkpoint 2 form.
Your student information should auto-populate.
Please use the table included to list all of your completed EdD coursework (including the semester, course number, course title, and final grade.
Click Next.
Step 3 - Type your name in the boxes - Click Sign Electronically
Your form will then be routed to your academic advisor for review, and then it will be filed as part of your student record.
