Repeat Course Surcharge Waiver Appeal
Students may appeal for a waiver of the state-mandated repeat surcharge as an exception for a previous withdrawal or failed attempt due to extenuating
circumstances of financial hardship only once for each course. (Previously attempted courses must have a final grade of a "W" or an "F" and extenuating
circumstances related to why "W" or "F" grades were earned). Appeals will be reviewed by the committee and decisions will be emailed to your UWF
student email account within 10 business days (excluding weekends and holidays) of receipt of a completed form.
Please Note the Following:
As the petitioner you must...
- Attach a typed letter describing the circumstances of the chosen exemption for the committee to review.
- Include all supporting documentation.
- Submit one waiver request per course.
- Submit your request within six (6) months after the completion of the course the Repeat Course fee is charge in ordered to be considered.
Please follow the steps below to submit your request to waive the repeat course surcharge through your MyUWF student portal:
Step 1: Enter "Repeat Course Surcharge" into the search bar found at the top of the page.
Step 2: Click on "Complete This Form" button.
Step 3: Fill out the requested information. (Please note, your name, student ID, student email address, and phone number will fill in automatically.)
