Options for building online forms

Creating surveys for research?

The Institutional Review Board (IRB) For Human Research Participants Protection must review and approve all surveys before users send them to respondents.

Google Forms	Anyone with an active UWF ArgoNet account (which includes active students and employees)	The simplest of the available form building options.	Training materials and support documentation for Google Workspace apps Do a word search for Google Forms. Active UWF employees may also visit the Professional Development Learning Library, to register for the trainings that the ITS Help Desk offers.
Qualtrics		More advanced question types, form logic, data collection, and reporting.	Accessing Qualtrics Introduction to Qualtrics training presentation Active UWF employees may register for Qualtrics trainings, offered by the ITS Help Desk offers.
DocuSign	Only UWF employees with active ArgoNet accounts	Allows for respondents to quickly sign PDF forms.	DocuSign Training Info For active UWF employees only. Employees must complete this training and submit the appropriate request, before being able to create and send out DocuSign forms.
