This page reviews the process for study abroad students planning to participate in USAC Programs to have the program cost confirmed by USAC.

Instructions

Login to MyUWF to Access the Study Abroad Consortium Agreement Form.

Search for and select the Study Abroad Consortium Agreement

Click the Complete This Form Button to Begin the form

Enter the Program Information Requested in the Form

Your Name and UWF ID will be autofilled in the form. You will need to confirm your Academic Major. Enter the Program Information requested. Be sure to enter information that matches your USAC program exactly. Enter the Program Name or Host University. Confirm the City and Country where you will study abroad. Confirm the Term (Spring, Summer or Fall) and the Year of the study abroad program. Enter the Starting and Ending Dates using the mm/dd/yyyy format. Dates must match the exact USAC Program Dates.

Scroll to bottom and select Next to move to the signature page of the form.

The Form will be sent to USAC for completion of the Cost of Education Information.

The USAC Staff member will confirm the Cost of Education for the program for which you have entered Program Information. Your UWF ID will be hidden from USAC Staff.

International Programs will review the form and provide a copy to UWF Financial Aid.
