Form 22-1990 This is the confirmation page page provided at the end of the application. Email it to us at mvrc@uwf.edu. Example of the required form: We will also need a copy of your Notice of Basic Eligibility (NOBE), which is part of your reserve contract. Example of required form: Note: You Notice of Basic Eligibility (DD Form 2384-1) is obtained from your unit. Your unit will also code your eligibility into the Department of Defense (DoD) personnel system so the VA can verify it. Register for Courses (if you have already registered skip this step) You will need to meet with your academic advisor to get registered for classes. After registering for classes, you will need to upgrade your MYUWF account to full access. To do this, type My account in the search bar of your MYUWF account. Click on Upgrade. This can be done at least 2 hours after registering for classes. Once the account has been updated, click the log out button. Once you log back into your account, you will have full access.
