Overview

This procedure explains how a student can view the status of their required immunization documents.

Instructions

Step 1 - Access the Immunization Status app 

Access the Immunization Status app via MyUWF by searching for "immunization status."

Step 2 - Review Messages

One of two messages will be displayed at the top of the page:

Complete

If all required documents have been received and processed, the following will be displayed: You have submitted all of the required immunization documents.

Incomplete

If any requirements are missing, the following message will be displayed: Your immunization records are not complete. Please note that a hold will be placed on your account prior to registration

All missing immunizations will need to be uploaded via the UWF Immunization electronic form, which is hyperlinked on the top of the page.

If you do not see a "UWF Immunization" link, it could be that you have not been offered admission to the university yet.  Once offered, check back to fill out the required immunization form.
