Repeat Course Surcharge Waiver Appeal

Under s.1009.285, Florida public institutions are required to implement a repeat course surcharge for students who take a state-funded undergraduate course (regardless of a student's enrollment level) for the third time. Students taking the same undergraduate course for the third time at UWF are subject to an increased matriculation fee of 100% of the cost of instruction.

Students may appeal for a waiver of the state-mandated repeat course surcharge. If you withdraw or fail a course due to extenuating circumstances, an exception may be granted only once for each course. Appeals should be submitted via the Repeat Course Surcharge Appeal Form.  Previously attempted courses must have a final grade of a "W" or an "F". Appeals will be reviewed by the committee and decisions will be emailed to your UWF student email account within 10 business days (excluding weekends and holidays) of receipt of the form.

Please Note the Following:

As the petitioner you must...

  • Attach a typed letter describing the circumstances of the chosen exemption for the committee to review.
  • Include all supporting documentation.
  • Submit one waiver request per course.
  • Submit your request within six (6) months after the completion of the course the Repeat Course fee is charged to be considered.

Please follow the steps below to submit your request to waive the repeat course surcharge through your MyUWF student portal: 

Step 1:

Enter "Repeat Course Surcharge" into the search bar found at the top of the page.


Step 2:

Click on "Complete This Form" button.

Step 3:

Fill out the requested information. (Please note, your name, student ID, student email address, and phone number will fill in automatically.) All red asterisks must be filled in including the course prefix and number you are requesting the waiver for, the semester and year each course attempt was taken, and the final grade earned. ("W" or "F")


Step 4:

Attach your letter describing the circumstances of the chosen course exemption for the committee to review and all accompanying documentation.

Your letter should be at least 200 words and include answers to the following:

  1. What contributed to you taking the course for a third time?
  2. Explain why the committee should approve your appeal.
  3. How would the approval benefit you?
  4. Provide any additional supporting documentation you feel will benefit your request.

Step 5:

Read the statement at the bottom of the form and check the box if "Yes". Click Next.

Step 6:

Read all information and sign the form electronically by clicking "Sign Electronically".

Step 7:

Your form has been submitted and will be reviewed by committee within 10 business days (excluding holidays and weekends).
