All applicants who are applying to the MSW program must submit a Field Practicum Form as part of their application. A link will be provided so that applicants can access the form, or applicants can search for "Master of Social Work Field Practicum" from MyUWF. Follow these instructions to complete the form:
- Your personal information should be automatically filled in. If these fields are not automatically filled in, please type your first and last name (as it appears in your student file), UWF student number, phone number, and email address.
- Select the term to which you have applied from the drop-down menu.
- Select the county where you plan to complete your required field placement hours from the drop-down menu.
- If you selected Escambia, Okaloosa, or Santa Rosa County from the drop-down menu, acknowledge the statement at the end of the form by clicking the checkbox. If you selected "Other," you must type the name of the county or city where you plan to complete your required field placement hours, then you must provide the requested contact information for three (3) potential field placement sites before you can acknowledge the statement at the end of the form.
- Once you have filled in all required information and clicked the acknowledgement box, click "next" at the bottom of the form.
- On the electronic signature page, type your name in the empty text box, and then click "Sign Electronically."
