Instructions

Step 1

DDC Chairs must log into MyUWF before completing the IPT Deliverable 1 and Advanced Standing Status form in Step 3 below. The form will not open properly if you are not logged in.

Step 2 - Please know the information listed below BEFORE opening the form.

  • Student's First and Last Name

  • Student's UWF ID Number

  • Student's Deliverable 1 Topic

  • Deliverable 1  Defense Date

  • DDC Members' Names and Email Addresses


Step 3 - Click HERE to access the IPT Deliverable 1 and Advanced Standing Status form.

Your name and email address should auto-populate.

Enter the student's first and last name, UWF ID#, D1 topic, D1 defense date and status, your comments (when necessary), and the names and email addresses for the committee members. 


Dissertation Committee Chair Name and Email Address fields automatically populated.

Step 4 -  Sign the form.

After the DDC Chair signs the form, it will be sent to the committee member(s) for their signature, (s), the Department Chair, Director of Doctoral Programs,  and the EdD Program Administrative Specialist for filing.
