Overview

Students use the Preferred First Name Request  ← ADD LINK form to declare a preferred first name. UWF recognizes students may wish to use a name other than their legal name. When requested, the university will use a preferred first name on certain documents and online information sources such as in the campus directory and on class rosters. The official/legal name will remain on the student's permanent records, including but not limited to academic, student accounts and financial aid.

UWF reserves the right to remove or deny the Preferred First Name Request if used inappropriately. This includes, but is not limited to, names using inappropriate language or names submitted to avoid a legal obligation or to create misrepresentation.

Step 2

Student's information will prefill at the top of the form. The student will need to read the disclaimer and check the box indicating their consent. The student will type their preferred first name in the box and submit the form. The Registrar's Office will process the form typically within 3 business days.

FAQs

 No. This is there as a service for people who wish to be known by something other than their primary/legal name. If you regularly use your primary/legal name then please do not enter a preferred first name.

Preferred first names will only be used in cases where the legal name is not absolutely necessary. Examples of where your primary/legal name is necessary include, but are not limited to, financial aid documents, student accounts, payroll, and official transcripts.

Any preferred first names that may be deemed by a reasonable person to be an attempt at misrepresentation, fraud, or interpreted as offensive will be removed by a member of the Registrar's Office staff. 

Complete the Student Name & Address Change form and submit it, along with the indicated appropriate documentation, to registrar@uwf.edu. Please note - This form does NOT change your address for PAYROLL purposes. Payroll addresses can be updated using the Contact and Privacy Information app in MyUWF or by completing Form W-4 and submitting it to the Payroll Office in Building 20E.
