Completing the Graduate Program Admission Acknowledgement Form

You will receive an email from the TEEL Graduate Advising Office with a link. Follow the link to access the form (pictured below).

1. Your name and student ID should be automatically filled in.
2. Carefully read each of the acknowledgement statements. Be sure to click all links and review the relevant information.
3. Click each box to acknowledge your understanding.
4. Once you have clicked all of the boxes, click "next" at the bottom of the form.

5. Type your name in the empty text box, and then click "Sign Electronically."
