Overview

This page explains how to submit the required documentation in order to verify the semester of graduation for the purpose of applying for Optional Practical Training if the Form I-20 needs ot be shortened before the student may apply for OPT.

Instructions

Log into MyUWF to Access the International Student Graduation Verification Form

You must be logged into MyUWF to access the International Student Graduation Verification Form. Once logged into MyUWF, click the linked text to open the International Student Graduation Verification Form.

Complete The Student Section of the Form

You will complete the student information portion of the form. See the example below of the student information. Name and UWF ID will autofill. You will need to confirm your academic major. Click Next at the bottom of the page to submit the form. You will sign the form electronically. 

UWF Registrar Review

The UWF Registrar will complete the Registrar Section of the form, verifying the expected date of program completion. This includes confirming if you have submitted an application for graduation and the semester (Fall, Spring or Summer) and year for which your graduation application has been submitted. The Registrar selects the next button at the bottom of the page and electrically signs the form. 

International Programs Review 

International Programs will review your form, shorten your program and issue a new I-20 with an updated end date if necessary. Students should simultaneously review and submit the OPT Start Date Request Form. When International Programs has completed the review and confirmed a new Form I-20 has been issued, they will click next and electronically sign and complete the form. You should maintain communication with International Programs while the form is pending and when it has been completed. 


Please note: It is important to for you to keep all Forms I-20 for your records. You will need them to apply for Optional Practical Training (OPT).
