Enrollment Certification for Exchange and Transfer Credit Programs
The Enrollment Certification form is used by UWF to confirm a student has arrived abroad and has begun classes. UWF requests each student Students will complete their personal information at the top of the form and submit to the host school or program international office after arrival. The host school or program office abroad may request will list the student to list their 's courses or modules on the form . UWF asks that each host institution or program's International Office and verify the course dates for each class and complete the signature portion of the form before sending the form to UWF. The Enrollment Certification Form serves as attendance verification for students abroad. Financial Aid cannot be disbursed until after the Signed Enrollment Certification is received by UWF from the host school or program abroad.

Student Information section of the Enrollment Certification Form
You must complete your personal information on the Enrollment Certification Form. To Complete the form at the start of the semester abroad, you should mark the line for "Initial Registration." Clearly type or write your Name, Certification Form including your UWF ID number, Last Name, First Name, Host School, and Semester and Year of Exchange.
Complete Your Class Schedule at Host School on the Enrollment Certification Form
You should enter the course details as noted for all of the classes you are taking at your host institution abroad: Host School Course Number, Course Title, and Course Level. This information should match the information provided by the host school. See the provided example for a host school course.
Submit the Enrollment Certification Form to the International Office of Your Host Institution or Study Abroad Provider
The International Office of your host university abroad will complete the Course Information at Host School Section.

The host university will need to add any additional information or comments for your courses. The designated staff member in the host institution International office will sign and certify the form.
Submit the Completed Enrollment Certification Form to UWF
As noted at the bottom of the form, UWF requests the completed Enrollment Certification form to be sent by email to registrarexchange@uwf.edu. The host institution abroad may email the form to UWF OR provide the form to you to send to UWF. The form must be submitted to UWF during the first week of the term abroad to prevent delays in release of your financial aid.
Submit the Enrollment Certification Form to UWF for ALL Course Changes Abroad
The Enrollment Certification Form is used to maintain your course registration while abroad. An updated form should be submitted for all course changes. Compelte the top section of the form as shown here for any course changes.
Course changes include schedule changes during drop/add at your host institution and course withdrawals after drop add.
Click "Next" and E-Sign the Enrollment Certification Form
Your Host School, the Office of International Affairs, and the Registrar will complete the rest of the form. At the bottom of the page, click the "Next" button to complete your electronic signature on the next page.
The Registrar will email you when the form has been completed by your host school.Submit additional Enrollment Certification Forms with Host Institution signature to registrarexchange@uwf.edu as noted on the form.
