Click HERE to access the EdD in C&I Program - Student Acknowledgment of Updates form.

Your student information should auto-populate.

Please carefully review the information and check the acknowledgment

boxes next to each update.

Click Next.

Step 3 - Type your name in the boxes - Click Sign Electronically

Your form will then be routed to

the Doctoral Support staff and filed as part of your student record.
