Students use this form to declare a preferred first name. UWF recognizes students may wish to use a name other than their legal name. When requested, the university will use a preferred first name on certain documents and online information sources such as in the campus directory and on class rosters. The official/legal name will remain on the student's permanent records, including but not limited to academic, student accounts and financial aid.

Instructions

Students will log into MyUWF be able to access this dynamic form . They will via either the Registrar's Office website or MyUWF. Both methods require logging in with your Argonet ID and password. Students will be able to complete and submit the form online. The Registrar's Office staff will process the form.

Please note - This form does NOT change your address for PAYROLL purposes. Payroll addresses can be updated using the Contact and Privacy Information app in MyUWF or by completing Form W-4 and submitting it to the Payroll Office in Building 20E.
