Overview


This procedure explains how to waive a Hepatitis B, MMR, Meningitis vaccine and/or the medical history form via the Immunization Status app. This only applies to those students who are fully online. Fully online means that the student's program(s) are fully online programs, AND the student is not living on campus, AND all courses currently enrolled in are fully online. This is all for the student's most recent term. The most recent term is the current term if they are enrolled, otherwise it will be the last term they attended or a future term if they are registered.

Instructions

Step 1

Log into MyUWF and search for Immunization Status.  Click the app.

Step 2

If you are in an online program and you haven't met all your immunization requirements, they can either be waived or taken exception on.  The following will appear to allow you to do so.

If "Yes" is selected for the upload docs question, you will be prompted to Proceed.  Once you do so you will be brought to the student view.

If "No" is selected for the upload docs question, you will be asked if you want to waive or take exception on at least one of the immunizations that are missing.

If "No" is selected for the waiving an immunization, you will have to click 'Proceed' which will bring you to the student view.

If "Yes" is selected for the waiving an immunization, you will have to click 'Proceed' which will bring you to a page that will allow you to waive one or all of the immunizations still needed.  In addition, if you haven't accepted the Medical Treatment Consent there will be a section at the top to allow you to do so without going to the Dynamic Form.

Once you select the apply checkbox that is directly under each immunization that can be waived, you will be shown the 'disclaimer' and the waiver request for that specific type of immunization.  Nothing can be saved until the "I accept" or at least one of the waiver request vaccines has been declined.

If save is clicked a message will be shown at the top of the page to let you know it's not valid.

Once at least one immunization is accepted and declined, you can now save.  When clicked, all the immunizations for the declined vaccine will be waived.  So if the Hepatitis B was waived, all the Hepatitis B vaccines that have not been met will each get a waived status applied to it.

Once the waiver or exception has been applied, you will be shown the student view with a message indicating the status.
