This page explains the process to submit the OPT Start Date Request Form.

Review the Application Guide and Resources for Optional Practical Training (OPT) Confluence Page

Before submitting the OPT Start Date Request Form, be sure you have reviewed the full information. 

Login to MyUWF for the OPT Start Date Request Form

After logging into your MyUWF account, enter "OPT Start Date Request" in the search bar and select the form name.

Start the form by selecting the Complete This Form button.

Page One: Student OPT Start Date Submission Page

Confirm you have reviewed the linked information and completed the steps listed prior to submitting your start date request. Click the box next to each statement. 

You will also select the date you are requesting to be listed as your OPT start date on your Form I-20.

Page Two: Statement of Understanding

Read the statement on Page Two and confirm you understand.

Page Three: International Affairs Confirmation of Review 

Students will select Next to move to the end of the form. International Affairs will complete this page when your form is reviewed.

Electronic Signature

Enter your name to sign the form electronically.

After Submission: Meet with International Advisor
